Case History: 18 year-old female with left shoulder pain

AP x-rays of the patients left shoulder in a standing position were normal. However, bilateral AC joint films with the patient holding ten-pound weights demonstrated widening of the AC joint and elevation of the distal clavicle. Question: What is your final diagnosis?
