53 year old woman with right foot pain

Case Presentation: Page 2 of 4 You should be considering metatarsalgia, infection, cellulitis, osteoarthritis, and inflammatory arthropathies.

Examination: T=98.1, P=84, R=12, BP=110/70 Neurologic exam of the lower extremities was within normal limits. The right foot demonstrated no warmth, redness or discoloration. Moderate was found over the dorsum of the second and third metatarsals and the second toe was swollen with ?sausage digit? appearance. A small scar was present over the dorsum from the hammer toe surgery and there was a scar over the first MTP joint from bunionectomy she had not mentioned during the history. Her gait was normal except for increased pain at toe off. Pes planus was present bilaterally. She was acutely tender over the second MTP joint and the second metatarsal head was subluxated inferiorly.

Question: What is your working diagnosis at this point? What other tests would you order?
