16 yo male football player presents with numb and tingling in right arm and weakening grip

Case History: 16-year-old male Defensive End Football Player presents with numbness and tingling in his right arm, neck, shoulder; and arm pain, arm fatigue during practice and sometimes during the school day as well as a weakening grip. The numbness and tingling in his arm does not follow a dermatome pathway nor does it follow a specific nerve pathway; it is more diffuse.

 

The patient states that his symptoms began after he took a “big hit” during summer football practice, 2 weeks prior, to the top of his right shoulder. He further states that his symptoms worsen during activity especially when he attempts to bring his shoulders back to straighten his posture or stretch his pectoralis muscles. He also states that symptoms are worse during the day at school especially when he is wearing his backpack. This patient has had no prior condition of any similarity or has had any other injuries except for “minor hits” on the football field which he does not consider a factor. He is not on any medication, does not smoke or utilize drugs. He is in football practice for 3 hours every day which includes weightlifting. When asked if weightlifting causes symptoms, he stated that dead lifts exacerbate his condition.

 

Question: Based on this clinical history, what are some of the differential diagnoses that you are considering for this patient?
