The diagnosis in this case was a Grade II ac joint separation. This is defined as the acromioclavicular ligaments being torn, with the coracoacromial ligaments intact but stretched. On normal x-rays the AC joint spaces should be symmetrical and within 2-3 millimeters of each other. 2-4 millimeters is an average range of absolute width. In terms of alignment the inferior and superior margins of the clavicle and acromion should be in smooth horizontal alignment. The distance between the inferior margin of the clavicle and the coracoid should be in the range of 11-13 millimeters with no more than 5 millimeters difference from side to side.
