This is a complicated case. We have identified the etiology of the back pain, multiple perineural cysts resulting in nerve root compression at least at the level of T10-T11 on the right. While perineural cysts can be an independent finding, they are also found in several systemic diseases such as Marfan syndrome, fascioscapulohumeral dystrophy and ehlers danlos syndrome. The presence of the patient’s prior history of early cataract disease, body somatotype and ligamentous laxity are suggestive of Marfan syndrome.
