The tropism on this patient renders the lumbar spine unstable. When she abruptly twisted to the right, the right sagittal facet of her tropism which usually limits her right rotation was forced beyond normal limits. Her injury from the joint locking initiated and maintained muscles in spasm. The gluteal muscles became abruptly hypercontracted and compressed on the sciatic nerve creating pain radiation pattern below.

 

This patient was successfully treated with spinal manipulative therapeutics to her lumbopelvic spine, sacroiliac joints and right greater trochanter. She also received trigger point ultrasound at the right buttocks near the greater trochanter to reduce the adhesions on her sciatic nerve linings from contracted gluteal muscles. Core strengthening exercises were also given to this patient.
