Case Presentation: Page 3 of 6

Standard functional testing of the knee includes valgus and varus stress of the tibiofemoral joint as well as anterior and posterior draw. The tibiofemoral joint was intact, not inflamed and no pain was produced in any of the functional testing.

Standard functional testing of the patellofemoral joint is mobilization in all directions including distraction, and compression with quadriceps contraction. All tests for the patellofemoral joint were negative.

Since this patient did not have positive findings in either of these joints, palpation of the associated muscles was introduced. It was found that this patient had a contracted lateral IT band at the knee, spasms at the quadratus femoris muscles and pain and spasms at the sartorius attachment to the medial knee.

Question: Which of the above mentioned muscles would cause limitation of movement and inability to bring this patient’s leg into a “figure 4”?
